About Us

Nepean Options uses a Person Centred Planning Approach to identify each clients goals and needs.

The Case Manager meets with the client and together they undertake a comprehensive assessment which will help them to identify that person’s goals and the areas of their life that they need assistance with.

The comprehensive assessment is then used to develop an Individual Support Plan that records the person’s goals and needs that have been identified. Every Support Plan is different because every person has different goals and needs.

The Support Plan clearly identifies the services needed, who will arrange them and when. The Case Manager then negotiates for the services to start and monitors them to make sure that everything is working as expected.

The Case Manager provides the client with a single person they can contact to arrange services they need and resolve any problems or issues that they encounter.
